• If Divorced: Decree Absolute
• If Widowed: Spouse's Death Certificate and Previous Marriage Certificate
• If Adopted: Adoption Certidicate
• If Name Changed: Deed Poll ( Also required for divorcees or widows who have reverted back to their maiden name)
